Individuals in the HCS/TxHmL program can attend the provider of individualized social skills service

Service Description

Individualized Social Skills Services Individuals enrolled in the HCS and TxHmL programs may choose any approved Individualized Social Skills provider in Texas. These services support community participation through leisure and recreational activities and help individuals acquire, maintain, and improve self-help, socialization, and adaptive skills needed to live successfully in the community. Service Locations (DBMD, HCS, TxHmL) • On-site • Off-site • In-home (HCS and TxHmL only) Examples of Activities On-site Activities may include: • Cooking and food safety (using stove, oven, dishwasher, following recipes) • Laundry skills • Money management (recognizing money, making change, writing checks) • Health and hygiene (personal grooming, oral care, hand washing) • Physical activities (walking, jogging, yoga, dancing, strength training) • Emotional regulation and mental wellness • Medication awareness, sleep, and relaxation techniques Off-site Activities may include visits to: • Libraries, restaurants, malls, and stores • Museums, aquariums, zoos • Bowling centers, coffee shops, senior centers • Morgan’s Wonderland • Volunteer opportunities in the community In-Home Individualized Skills & Socialization Criteria One of the following must apply: • A physician documents a medical need for in-home services • A licensed professional documents behavioral needs requiring in-home services • The individual is 55 years or older and requests in-home services In-Home Service Provision • HCS and TxHmL providers must deliver in-home services at the individual’s residence • In-home services are not required to be provided by an Individualized Skills and Socialization provider
